Have you ever been convicted of, plead no contendere (not contest) to, or received a deferred or suspended sentence for a crime more serious than a parking offense (including speeding tickets) in this or any other state, territory or country? If yes, list the date, offense, and judgment on a separate sheet of paper, sign and date it in a sealed envelope with your name on the outside of the envelope and attach to the back of the application. Be advised that answering “yes” will not necessarily bar you from admission to student teaching or certification. Teacher candidates will be placed at school districts located within sites. These four sites are: Sioux Falls, Sioux City, Vermillion, & Wagner. Each site has multiple school districts that surround or are in close proximity to the site in which the candidate may be placed. All residency placements are made collaboratively with the school districts. A map of the sites and list of school districts for each site are located on the following Google page:
Please list your site preferences by identifying your first, second, and third choice. Provide a rationale for your preference and if no preference, check box below. Stating a preference with rationale does not guarantee placement. Each preference with rationale will be considered on an individual basis.
Check this box if any scholarships and/or requirements must be fulfilled during residency year. Specify

As a non-traditional student, I don’t have the flexibility that other traditional students have. I own and live in my home in Sioux Falls with my significant other and daughter. Sioux Falls is where we have our full-time careers and my daughter goes to daycare, which will be preschool by the time I begin student teaching. Commuting three days per week to Vermillion has already put an enormous burden on us in terms of time spent driving (average for me is about 7.5 hours per week; nearly a full work day!), wear and tear on our vehicles and costs of vehicle gas and maintenance incurred from the significant increase in mileage driven per week (about 375 miles per week!), and time spent at home with each other due to spending nearly all time either at campus, driving, or putting in a now varied and often late-night work schedule to maintain my current full-time career choice. Being in Sioux Falls would simply allow me to focus my efforts more on the school and learning as opposed to needing to be constantly driving to and from one work place to another; also, with less commute time, it will be easier to continue to also maintain my current full-time career, a must necessitated by the cost of owning a home, having a family, and, well, being an adult.
Note: I’ve also been encouraged by several educators both outside of and within the Harrisburg school district to ask about getting into the Harrisburg school district. I know several of the educators and so the support offered within their system would be immediately exceptional; the district is near enough to fit ideally into my current work and home schedules; and I would simply be most comfortable there, allowing me to focus my efforts on learning and being the best educator I can be as opposed to placing any emphasis on becoming comfortable with my surroundings and fellow staff.
