Action Plans for completing standards.
I have to ensure I am incorporating my standards into my work routine and I am therefore on the lookout for situations or opportunities which give rise to CPDS that I can use for my performance standards portfolio. I need to be more involved in clinical interventions and recording the outcomes of what intervention I have made; e.g. with regards to my amlodipine CPD; sending a fax to GP would have been more appropriate instead of simply referring the patient. If I see a patient on many medications, suffering from interactions I need to address the best action to minimise patient inconvenience. I deal with many Rx queries daily and so need to be aware of any situations whereby I might need to inform doctor of an alternative product due to interaction, or treatment option.
I need to prepare a presentation to my colleagues on the topic of inhaler technique. This will require me to watch the podcast on the CPPE website and take notes. I will outline the service and present to my colleagues the benefits of the service to the health economy. I want to try and engage my colleagues and try to work out with them how we are going to help improve our patient’s technique and provide ideas as to how we are going to help our pharmacy engage patients. I will then connect this to identifying patient’s technique of inhaler usage and see if they are in fact using the correct technique and workable options to resolve the problem.
I need to make more use of the consultation room with patients, e.g. with sensitive issues and I need to carry out a COSHH assessment in our pharmacy. Mentoring work experience students will involve me devising a plan for them so they can follow this, throughout their two week stay. I will act as a guide for them, should they have any queries.
I need to make a graph based on the collection of my questionnaires in the pharmacy and submit my results to my colleagues and discuss the results with them, seeing if there could be any strategies to improve patient’s satisfaction.
Finally I need to take time to attend branch meetings and locally run courses in the area.
By June 28th I want to have completed my inhaler technique using the podcast and began my presentation so that I can focus on more interventions during work hours and try to begin counselling patients on technique. I also want to have submitted my evidence and work via graphs to my colleagues and discuss improvement strategies.
By July 4th I want to have carried out both my presentation and COSHH assessment and I want to have carried out at least 10 inhaler techniques with patients by the 15th July.
By the end of July I want to have enrolled and attended at least two locally run courses/branch meetings.
