Re: Representation for implant surgery related infection
Dear Ms Usakowsky:
I am responding to a letter I received from your office asking for my assistance in obtaining medical records for a possible case. You indicated it could expedite matters if I tried to obtain them on my own.
I already had in my possession medical records from 12/21/2012 through 6/2013. After that I was referred to a doctor in Chicago, IL. I do not have medical records for those visits. While I did have the MRSA infection when I first had a consultation with Dr. Finn at Weiss Hospital he was not responsible for the infection. He could however confirm that I had the infection when I first saw him. After changing doctors and going through more infusion therapy and having two more surgeries, I did not get the infection again. However, if you feel these records are also necessary I will try to obtain them for you.
I am enclosing the medical records I do have and if I can be of further assistance, please advise. If possible, when you have finished reviewing the documents I would appreciate you sending them back to me. They are the only copies I have and had to pay to get them.
Thank you for your time.
