Fill out the first paragraph of the for by stating your are writing the court on behalf of, fill in the blank on the form.
Introduce yourself in the first paragraph and state your relationship to the defendant and how long you have known the defendant and or his/her family.
In the second paragraph include any accomplishments that the defendant has completed. For example you can state that the defendant has completed a drug class, obtained a GED or High School diploma or any college hours or classes the defendant has completed. You may also include anything the defendant has done for the community and any other reason why you think the court should show leniency on the defendant in this case.
In the third and final paragraph, you should thank the court for reading your letter and simply print and sign your name at the bottom.
If you are not going to be preset at the time the notes will be read by the court, you should have the document notarized. This should not cost more than five or ten dollars.
