This is the progress of my sleep apnea issues concerning my doctors and using the sleep apnea machine
Feb 23 Dr Delacruz visit due falling asleep while drive & extremely tired all the time
3/5-7 at home sleep study performed, sleep study repot date 3/8
March 15 Dr DeLaCruz to confirm sleep apnea confirmation
4/14 ENT Dr Hekiert: lots of issues with sinues
4/16 Princeton Radiology for CT of sinues
April 18 Dr Patel ordered sleep apnea machine
5/10 Respcare: Randy explained machines & checked breathing and had allergy tests & breathing test for asthma performed
May 16 Respacare Dr Patel went over test results
May 18 Wed tech(Jermaine) from SleepCircle visited to fit me for machine. The only mask that fit me was a small unit to fit slightly into and below my nose.
Sat May 21st started to use machine ( waited for weekend due to not knowing how I will react & if able to sleep)
July 25 5:45 Respacare Dr. Patel routine visit & was told me sleep apnea report was looking good
July 28 ENT Dr. cauterized nose
July 29 called Sleep circle to tell them can’t use machine due to nasal issues and not allowed to blow my nose for 2 weeks
July 30 Robert Wood Johnson University hospital, serious nose bleed from both nostrils, both eyes & going down my throat. Took over 4 hours to get my bleeding under somewhat of control & Dr. packed my nose. Telling me to be careful not to touch or bump it. So I slept on the recliner.
Aug 1 ENT Dr. Hekiert to get nose repacked
Aug 11 ENT to get nose checked & cleaned out somewhat, still can’t blow nose too much and was told to go easy/gentle
I continued to sleep on recliner from July 30 to Aug 13th still due to nasal issues
Aug 14 nose is better so back to using machine
10/10 Dr Patel told me the sleep study report showed me in 100% compliance
Why period July 2 to Aug 9th
This is what occurred upon receiving letter dated Friday, August 12, 2016 from SleepCirle as Final Notice.
First this was the only notice I ever received.
Aug 16 called Kathleen Lawton at Horizon who told me to contact CareCentrix. Then talked to Malain at Care Cebtrix, and Josie at SleepCircle who said she will contact compliance and to continue treatment.
Aug. 17: talked to Karen at Care Centrix who put me on hold while she contacted Josie to tell her to go ahead and bill for Aug. after understanding my sinus problems which had prevented me from using the machine from July 30th to Aug. 14th.
10/3 called SleepCircle to see what happed to my supllies. Talked to Kim.
10/5 talked to CeCe in the IN Take Department at 9:15 who told me to talk to a manager (Donna).
10/5 Talked to Donna at Care Centrix Ext 1703 who called me back at 11:30 to tell me that the authorization department was working to fix this and I should then get my supplies.
Friday Oct 14th I received 2 notices dated on 10/7/16: one for Code 4560839 E0562 CPAP Humidifer, Heated,
and the other notice: Code 4560911 E0601 Continuous Positive Airway Pressure Device (CPAP)
Both notices are basically saying that my Doctor must call within 3 days to establish medical necessity. This is due to not meeting by compliance which was caused by the nasal issues detailed above.
I have attached copies all the notices.
I will contact Dr. Patel and ask him to please call the Utilization Management Department to discuss this case before Wed. the 19th.
I feel that 3 days to get any busy doctor to reply in 3 days is not a feasible amount of time.
I have made so very calls to help you understand the reason I was not able to use the machine for certain days. I feel that this request for my doctor’s response in 3 days is not adequate time.
